This Waiver and Release of Liability (the “Waiver”) is executed by the undersigned participant or the participant’s parent/legal guardian if under 18 years of age (collectively, the “Participant”) in favor of OnFrameSoccer, its owners, directors, coaches, and facilities used (collectively, the “Released Parties”).
The Participant acknowledges that participation in soccer training involves physical activity, potential contact with other participants, and inherent risks including, but not limited to, falls, collisions, weather conditions, and equipment failure. The Participant voluntarily assumes all such risks associated with participation in the activities.
The Participant hereby releases, waives, and forever discharges the Released Parties from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in the training sessions or while on the premises where the training is conducted.
The Participant authorizes the Released Parties to act on their behalf in seeking emergency medical treatment in the event of an accident or injury. The Participant agrees to be responsible for all costs associated with such treatment.
The Participant grants permission for [Your Business Name] to use photos or videos taken during training sessions for promotional or marketing purposes, without compensation or further notice.
The Participant acknowledges that they have read this Waiver and understand its terms. They understand that they are giving up substantial rights, including the right to sue. They sign this Waiver freely and voluntarily.
